➤Medical treatment, applicant have to submit copy of Bangladeshi hospital treatment and Recommendation letter from local doctor (BD).
➤Appointment letter from the India hospital .
➤Passport with validity not less than 6 months from the time of application.
➤One set of passport photocopy page 1-5, correction page, Last India Visa Copy  if any. Renew copy
➤2 copy (50X50) mm size color picture with white back ground (no cap or sun glass). 
➤Update Bank statement for the last 6 months (balance more than 30k) or  Endorsement  200 USD
➤Leave letter from the office if employed or student and must official ID card copy. 
➤Trade license copy Notarized if (businessman).
➤Company pad and visiting card.
➤If Child then Birth certificate or school ID card copy. 
➤Parents’ consent letter for applicant aged under 18.
➤Utility bill copy (water, Gas, Telephone, Electricity)
➤NID card copy.
